Welcome to Woodward Public Schools
We are glad you have chosen Woodward Public School to educate your child(ren). All students new to Woodward and former students who have moved and are returning to our district must complete the online registration listed below. Once your application has been processed you will be contacted by the school your child will be attending. Please remember to take the following information that is needed with you.
State Enrollment Requirements:
Per State law, no child shall be enrolled in Pre-Kindergarten unless he/she has reached the age of 4 years on or before September 1 of the school year.
Per state law, no child shall be enrolled in Kindergarten unless he/she has reached the age of 5 years on or before September 1 of the school year.

The following documents are needed:
State Birth Certificate: (hospital certificate is NOT accepted). If you do not have your child’s state birth certificate you can click the link provided to get one ordered.
Oklahoma Vital Records or US or Abroad Records.
Proof of residency: gas, water, electric bill, or a signed lease agreement that is current within the last 30 days. If the applicants are currently living with someone else you may use one of the approved items above with their signature and statement that you are currently residing at that address.
You may also use a current DHS documentation in the parents’ name.
Immunization records: All students are required to have the following shots listed below as required by grade.
Pre-kindergarten – 4 DTaP/, 3 Polio, 1 MMR, 3 Hepatitis B, 2 Hepatitis A, 1 Chickenpox/Varicella
Kindergarten - 6th Grade – 5 DPT/DTaP*, 4 Polio*, 2 MMR, 3 Hepatitis B**, 2 Hepatitis A, 1 Chickenpox/Varicella’
7th-12th grades – 5 DPT/DTaP* + 1 Tdap booster, 4 Polio*, 2 MMR, 3 Hepatitis B**, 2 Hepatitis A, 1 Chickenpox/Varicella
Parents’/Legal Guardians’ photo ID; You will need your identification with you.
Items needed if applicable:
Special Education (IEP)
Legal Court Documents (name change, custody agreement, etc.)
Tribal Citizenship card or CDIB Card (Certified Degree of Indian Blood)
If the move is during the school year the following would be appreciated
Withdraw grades (k-8th)
Unofficial High School Transcript (9th -12th)

In order to help serve our community better Woodward Public Schools is now working in collaboration with the Big Five Head Start Program. We are asking Prek4 parents to fill out the application for Big Five after you have filled out our registration form. If your child qualifies for services through Big Five you will be contacted by Big Five for additional information to begin services.

2026-2027 Enrollment
NOTE: To parents of students that finished the 25-26 school year there is no need for you to fill out an application form. We are not doing an existing student registration for the 26-27 school year. Your students have been rolled up to the 26-27 school year. If you have any information that needs to be updated please contact your child's school. If you have more than one child in school you only need to contact one of the schools to get your information updated.
