For enrollment, all parents need to bring your child's shot records, proof of residence (ex. water bill, gas bill), and proof of guardianship along with each of the appropriate forms below.  Click on the appropriate forms under Name, then click on Download Now!, and print a copy of the forms, fill them out, and return these to your child's school. 
All new students being enrolled in the district need to have the forms from the New Student Enrollment Packet as well as the downloads from the appropriate Enrollment Packet.

NOTE:  ALL elementary students, grades Pre-K through 5th, will occur on Thursday, August 5, 2010 from 8:30 AM through 3:00 PM ...and... on Friday, August 6, 2010, from 8:30 AM through 12:00 PM (noon).  Enrollment for all these grades will be held in the Middle School Cafeteria located at 914 Oak Street.  ALL Pre-K through 5th grade students anntending elementary school in Woodward must enroll on these days, even if pre-enrollment papers were filled out last Spring at their individual schools.  Please remember to bring your child's birth certificate, immunization records, and proof of residency such as a water bill.
